Plasma Cleaning has proven to be at least as effective as ultrasound baths in removing particle contaminants from finished posts, and microbiological analyses have shown the total absence of bacteria on the decontaminated surfaces, even at implant-abutment connection level. The use of Plasma Cleaning is therefore able to favour the healing of the soft tissues and to improve the contact and stability among connections.

The plasma cleaning decontamination technique, carried out chair-side with the PLASMA-R unit, is highly effective in removing laboratory processing contaminants from implant posts, and is able to prevent the interference of processing residue with the biological stability of the tissues.

This studio demonstrated that, no matter how implant posts are treated during the technical preparation, they present contaminations which could interfere with the biological stability of the tissues. The conclusion is that is advisable to follow suitable cleaning protocols.

The authors measured the removal torque of post connecting screws decontaminated with plasma cleaning They found that the complete cleaning of post connections improves the pre-load of the screws, minimising loosening, in particular using new screws at the moment of securing. In conclusion they indicate the advisability of using screws and posts free from residue, that is which have undergone an adequate cleaning protocol, preferably new ones.

ABUTMENT SCREW REVERSE TORQUE VALUES BEFORE AND AFTER PLASMA CLEANING

After having checked the torque values for the removal of screws that had been treated with steam cleaning and of screws decontaminated with Plasma Cleaning, the latter showed higher reverse torque values, with consequent lower risk of loosening during operation. To reduce the loosening of screws during the final securing of the post, the authors therefore recommend following an accurate cleaning protocol that is able to eliminate residue from the threaded parts of the screws.

In the literature it has been demonstrated that when an implant is placed in the oral environment, changes in peri-implant hard tissue level may occur. Several clinical procedures have been advocated to clinically minimize this bone resorption. However, minimal attention was paid to the soft- and hard-tissue abutment interaction. The present clinical preliminary study is intended to radiologically analyze the effect of glowdischarged abutments on hard tissue level changes after 18 months of prosthetic loading.
Five patients needing an implant supported restoration in the anterior maxillary area with thin gingival tissue biotype and healed bone were recruited. An implant was inserted and a titanium abutment was glowdischarged with an Argon plasma treatment in a plasma reactor and, immediately after, definitively screwed at 30 N. Provisional restoration was therefore positioned. Three months later, definitive restoration was performed. Digital periapical standardized radiographs were taken at the time of surgery, 6 and 18 months thereafter. Radiographic analysis was carried out using image analysis software. At baseline, interproximal radiographs revealed no bone defect around implants in both groups. After 6 months from baseline, the postoperative interproximal radiographs revealed an average bone loss of 0.09 mm (range 0.0–0.3 mm, SD = 0.144 mm). After 18 months from baseline, the periapical X-ray showed a stable condition of bone remodeling (mean value: 0.09, range 0.0–0.5 mm, SD = 0.08 mm). Within the limit of this study, glow-discharged abutments have been demonstrated to positively affect hard tissue reaction to implant restoration.

During this interesting clinical research, the authors ascertained that using the One-Abutment-One-Time technique, together with the use of the plasma cleaning technique to remove particle and microbiological residue from posts and their connecting screws, has a positive effect in the reaction of the peri-implant hard tissues, the remodelling of which remains stable at 18 months.

The authors investigated the behaviour of peri-implant tissues following chair-side treatment with plasma cleaning of the posts and connecting screws. The research showed that the crestal bone is maintained significantly better around implants loaded with posts that have undergone this process rather than ones that have not been treated.

This publication confirms the preliminary results presented at the EAO 2013 Congress. Plasma cleaning is effective in the preservation of peri-implant bone levels.
