IMPLANT-PROSTHETIC REHABILITATION OF THE TWO ARCHES WITH IMMEDIATE LOADING: WORK PROTOCOL WITH THE USE OF THE SINGLE-MODEL AND P.A.D. SYSTEMATICS

In this clinical case, the single-model technique allowed rehabilitating the patient with one surgery and a minimal number of sessions for fi nalising the screwed prosthesis, obtained with the CAD-CAM Echo protocol on Kohno implants.

ZIRCONIUM PROSTHESIS ON IMPLANTS AND NATURAL ELEMENTS

The implementation of the Echo prosthetic protocol allowed obtaining an entire meso-structure in zirconium with excellent functional and aesthetic characteristics; the structure is also suitable for maintaining the morphostructural integrity of the remaining teeth.

SOFTWARE-ASSISTED SURGERY WITH THE GUIDE MODEL TECHNIQUE: 3D PROJECT ? PLACING OF THE GUIDED IMPLANT

The use of software for assisted surgery allows the three-dimensional diagnosis of the bone volumes and correct planning of the implant insertion, taking full advantage of the anatomy of the existing bone. This case includes the insertion of highly angled implants on which was constructed a Toronto screwed prosthesis using PAD abutments.

The extremely wide use of implants leads to more frequent complications and failures. General research and the numerous contributions made by Professor Covani’s research team have highlighted the excellent prognosis of immediate implants and their immediate prosthetization. Therefore, it was obvious that Professor Covani’s team should examine the theme of immediate replacement of the implant, when they meet complications that otherwise could not be treated. This subject has already been dealt with by the author and today, with this new publication, paves the way not only for immediate replacement, but also for immediate prosthetization. Pending large-scale studies, the authors present the case report of a 58 year-old non-smoking male, who had an implant that is no longer osseointegrated due to an infection. Since the patient refused to use a removable provisional prosthesis, the possibility to apply an immediate loading protocol was considered. The implant failed and it was carefully removed. The post-extraction socket was curetted to completely remove any trace of remaining granulation tissue. The prosthetic treatment then began with the patient still under local anaesthetic. By taking advantage of the great initial stability of a Premium implant (Sweden & Martina), excellent results were obtained in
both functional and aesthetic terms. This protocol – which does not require the element to be placed in occlusion in order to minimise the masticatory load until healing – allowed a perfect modelling of the tissues around the implant and a full recovery of the damaged site.

EARLY LOADING OF DENTAL IMPLANTS SUPPORTING A FIXED PROSTHESIS IN THE POSTERIOR EDENTULOUS MANDIBLE

The case report aims at presenting the clinical results relating to early loading after six weeks of the implant intended to support implant prosthetic rehabilitation in the posterior edentulous mandible. The implants were placed in sites 45-46 and loaded early with signifi cant psychological and functional benefi ts for the patient. This method undoubtedly offers advantages to the doctor by reducing treatment times and discomfort to the patients, thanks to the help of laser therapy, and also advantages to the same patient by reducing psychological and functional discomfort related to being partially edentulous.
The smaller number of failures and stability of the hard and soft tissues around the implant lead to the assumption that this new concept is successful for treatments on the lower jaw.

IMPLANT-SUPPORTED REHABILITATION OF BOTH EDENTULOUS JAWS WITH IMMEDIATE LOADING

In May 2011, a patient aged 69 years presented for generalized mouth pain, and to be able to assess the stability of the existing prosthetic work carried out by another dentist. The general medical anamnesis was negative. The patient declared that he had had several teeth extracted due essentially to caries, which were substituted with a number of implants some years before.
On objective examination, the situation of oral hygiene was very poor. No lesions were noted to the oral mucosae, and neither were there alterations to the main lymph node stations. In greater detail, intraorally it was possible to observe a skeletal class III malocclusion with an inward bite. The presence of periodontal disease affecting all the remaining teeth was also evident, with level 2–3 mobility on all teeth and some implants. After impressions, waxes and the facial arch were taken, the models were fitted to an articulator. The required wax-up was constructed using facial photographs.
A preliminary treatment plan was then formulated, with the aim of alleviating the pain and rehabilitating the mouth:

• Extraction of all teeth by now impaired and removal of the implants without adequate bone support.
• Cleaning of treatable implants (16, 17, 33)
• Positioning of upper and lower mobile prostheses to permit healing of mucous and bone tissues in keeping with the diagnostic wax-up.

41 patients received 70 Premium implants inserted with a flapless procedure versus opening of total-thickness flaps. Cumulative survival at 24 months was 98.57%, and with an average resorption of 1.19±0.9 mm.
Bone loss at 24 months was significantly higher in the group with total-thickness flaps. The preservation of marginal bone level with Premium implants was excellent.

ASSESSMENT OF A CLINICAL AND TECHNICAL PROCEDURE FOR THE REHABILITATION OF EDENTULOUS JAWS

This series of clinical cases with Premium implants, inserted with a protocol of guided surgery (ECHOPLAN) and rehabilitated with temporary immediate loading using the SIMPLE technique, and subsequently definitively with Toronto Echo made using CAD/CAM methods, validated the quality and replicability of the entire clinical procedure based on a digital approach.

BILATERAL REHABILITATION WITH GUIDED SURGERY

Negative anamnesis for pathologies or situations with contraindications for implant insertion. Complex dental anamnesis.
Amongst other procedures, it was necessary to extract element 36 located post-distally from the 34–36 bridge.
To resolve the bilateral lower rear edentulism, 4 Premium implants (Sweden & Martina) were planned with immediate loading, with 36 being inserted immediately post-extraction.
For details on the dimensions of the implants and their positions, see chart 1.
Flapless surgery was performed with a Surgiguide guide (Materialise) with dental and gingival support, and Echo Plan (Sweden & Martina) surgical kit, following the sequence indicated by the surgical manual.
The temporary crowns for immediate loading were made pre-operatively on an Immediate Smile model and passivated with intraoral gluing.
