IMMEDIATE TEMPORARY CROWNS ON GLOBAL IMPLANTS (CLINICAL METHOD)

As part of a series of in-depth studies on prosthetic protocols, two clinical cases are presented that use the mounter from the Global implant line as a temporary abutment to make the  immediate temporary crowns, with signifi cant time and cost saving. The Global implant system is a screw fi xture with conical
body and double octagonal internal connection. The outstanding precision of the abutment and the special patented connection make it an excellent implant for single-tooth replacement. An important feature is that this implant is positioned in the surgical site with a mounter that, if left connected to the actual fi xture, acts as a provisional abutment: it is suffi cient to place it on top of a special dedicated cap in resin and take an impression with traditional materials for obtaining a temporary prosthesis to be
cemented. The immediate temporary prosthesis was  manufactured by following the characteristics of resistance, passivation and accuracy with reduced execution times (within 4 hours). The double octagonal connection revealed to be extremely retentive and no disconnection between the implant and abutment was verifi ed in 300 of the cases treated over a period of 36 months. The final crowns in metal-ceramic or in zirconium-ceramic were cemented with hardening auto/photo composite cement.

OVOID PONTIC IN IMPLANT PROSTHESIS

Implantology now represents a valid alternative to traditional prosthesis with dental support, and is often the only alternative for rehabilitating a fi xed prosthesis if there are not a suffi cient number of natural posts. It should also be remembered that it is increasingly more common, even in intercalated edentulism, to try to preserve the natural undamaged elements, avoiding the preparation of the post, by inserting fi xtures in the adjoining edentulous areas.
For years, the clinical validity of replacing natural teeth with osseointegrated implantology has been demonstrated to be a valid solution.
The challenge in the last few years has come from camoufl aging the prosthesis on implants between  the natural teeth in aesthetic zones. The loss of natural teeth can occur for periodontal, endodontic or traumatic reasons; in all cases, bone reabsorption can occur, and with it the loss of the normal gingival shape.
This work presents some routine clinical cases, which on one hand, propose a pre-prosthetic method aimed at better integration of implant prosthetic rehabilitation in the oral cavity of patients, and on the other hand, a now more common method aimed at pushing the soft tissues, guiding them with the provisional implant, whether on the teeth or on the implants.

This finite element analysis suggests that a Toronto prosthesis is capable of better distributing the stress on six fi xtures, rather than on fi ve, and that it is always advisable to position two implants intraforaminally and instead of only one implant.

This study on finished elements compared the results of the application of stress cycles on Global implants rehabilitated with single screw-retained crowns versus cement-retained crowns. The results showed that screw-retained prostheses exert greater stress on the tightening screw than cement-retained prostheses.

IMMEDIATE IMPLANTS IN EXTRACTION SOCKETS. CRITICAL FACTORS IN THE HEALING OF HARD AND SOFT TISSUES

This further review of the literature considers 13 articles (4 of which with Sweden & Martina implants) which analyse the morphological variations of the hard and soft tissues after the insertion of immediate post-extraction implants. The review concludes that surgeons should always consider the risk of technical complications, which are foreseeable when the thickness of the vestibular bone wall is less than one millimetre. In all these cases, if a high aesthetic value is necessary, the review advises waiting 6/8 weeks after extraction before inserting the implants and preserving the site with suitable regenerative techniques.

This extensive systematic review of the outcome of immediate post-extraction implants was drawn up during the 6th Expert Meeting of Osteology, with the title Evidence-based Knowledge on Biology and Treatment of Extraction Sockets Including the Placement of Dental Implants. Of the 46 works included in the review, 9 concerned the use of Sweden & Martina implants, which are therefore rated amongst the most highly referenced worldwide. According to the review, the studies included all demonstrate that the survival rate of post-extraction implants is high, while in other studies the results are sometimes not in agreement as regards the success parameters (stability and maintenance of peri-implant tissues). Further studies are therefore necessary to investigate these aspects.
